You should notify the relevant NHS Board about any changes to your details within seven days of the actual amendment. This is to comply with 2016 regulations from NHS General Ophthalmic Services in Scotland.
Amending personal information
Contact your main host/health board if you need to make changes to your personal information. This might include your name or address, for example.
Amending listing arrangements
If your amendment involves the listing arrangements then you need to contact the board where the amendment is to take place. This may not be your host board. Examples include:
- transferring to another part of a list
- a change to practice details
- resignation.
