Access GOS6A forms

The GOS6A forms are only to be used by NHS boards and we will only accept them when submitted by an NHS board authorised signatory .

How to submit forms

Once the form has been completed, an authorised signatory should submit it to NSS.psd-customer-admin@nhs.scot from their personal NHS email account, with 'GOS6A' in the subject field.

Please note that paper forms will not be accepted.
