Practice mandates can be used to advise us where to send information or make payments. They also let us know who has authorisation in the GP practice to sign and approve registration or payment documents.
The following mandates are available on-line:
GP Practice Authorised Signatory Mandate - advising Practitioner Services of all medical and clerical staff authorised to sign and approve registration and payment documents within the practice. Please complete this mandate on-line by either typing into the fields in your browser or downloading the file and opening in Adobe Reader DC to fill the editable fields. We are moving away from hand written mandates and have provided a means of completing by typing which is what we have been asked for.
GP Practice & Dispensing Doctor Bank Mandates advising Practitioner Services of the bank account to which payments are to be made.
GP Practice e-Statement e-Mail Address Mandate -advising Practitioner Services of the e-mail addresses to where the e-Statement should be sent.
