Find out how and where to submit your ophthalmic mail

Published on 01 May 2022

No ophthalmic mail should be sent to Bain Square. No paper GOS payment forms can be processed. If mail is sent to the wrong address this will delay the enquiry being dealt with or payment being made.

Any paper forms received by post after 01/05/2022 will be returned for resubmission by email, the only exception being HES (Hospital Eye Service) forms, which will continue to be submitted as paper copies.

Hospital Eye Service (HES) Forms

Post HES1, HES3 and HES4 forms to:

(D&O Operations department)
Practitioner Services
Gyle Square
1 South Gyle Crescent
Edinburgh, EH12 9EB

Payment for postage

If practices underpay or do not pay for postage when sending HES forms, we will not accept these items. Any mail which does not have the correct postage paid will be refused and returned by Royal Mail if there is a return address.

From 01/05/2022, the following forms must be submitted electronically by email from a nhs.scot email account:

E-Ophthalmic (e-GOS) set up:

Ophthalmic online report (e-schedules) forms:

  • 1201 - Online reporting registration form;
  • 1202 - Close an online account;
  • 1203 - Amending payment location codes on your existing user account.

All forms types detailed above should be submitted via e-mail from a nhs.scot email account to NSS.psd-customer-admin@nhs.scot, any queries received from a non nhs.scot email account will not be processed.
