This is a freedom of information request seeking access to the 42 points of concern identified by NHS Assure with regard to the new Orthopaedic ward extension being built at Forth Valley Royal Hospital. I would also appreciate the names and qualification of those within NHS Assure who assessed the layout.

I can advise you that we have now completed the search of our records and can provide you with the following information:

This is a freedom of information request seeking access to the 42 points of concern identified by NHS Assure with regard to the new Orthopaedic ward extension being built at Forth Valley Royal Hospital.

Please find attached Appendix 1 - 2022-05-17 NHS Forth Valley FBC KSAR Report V2.0, for the information in respect to the 42 points of concern noted by NHS Scotland Assure in a Key Stage Assurance Review (KSAR) in relation to Forth Valley Royal Hospital.

Following issue of the report to NHS Forth Valley, the health board prepared an action plan outlining how they planned to address the findings. Since the issue of the report, NHS Scotland Assure have worked with the health board to review their progress and will continue to do so across the remaining stages of the project. Further KSARs will be undertaken at the Commissioning and Handover phase of the project where the health board will be required to provide assurance that all key findings have been addressed. Details of the process are outlined within DL(2023) 03 issued by Scottish Government on 6th February 2023.

I would also appreciate the names and qualification of those within NHS Assure who assessed the layout.

The KSAR focuses on key topics, specifically – IPC, water, ventilation, electrical, plumbing, medical gases installations and fire. All KSAR reports are reviewed and approved prior to issue by senior NHS Scotland Assure technical staff. Specifically either the Assistant Director for Engineering & Assurance (Ian Storrar BSc (Hons) CEng FCIBSE FIHEEM MIET) or the Head of Engineering (Thomas Rodger MEng CEng MCIBSE MIET FIHEEM).
