Registration Partners Forms

Quarterly and annual declaration forms for completion by GP Practices who no longer submit paper GPR forms to Practitioner Services.

These apply to the electronic transfer of patient data. Your GP practice no longer needs to complete this exercise as this Scottish Government process was discontinued in August 2025. This applies to the following two declarations at the appropriate points in the year:

quarterly confirmation - 30th June, 30th September, 31st December & 31st March annual agreement - 31st March
