Application form and guidance for those with chronic and advanced hepatitis C seeking to join the Scottish payment scheme.

Details
This form is for applicants who've never joined the Scottish Infected Blood Support Scheme, or any of the UK Schemes (e.g. Skipton Fund). It's for hepatitis C payments for eligible candidates who were:
- infected by hepatitis C as a result of treatment they received themselves with NHS blood, tissue, or blood products
- infected by hepatitis C as a result of the virus being transmitted from someone else, who themselves were infected by hepatitis C as a result of treatment they received
It contains guidance notes and the application form K for printing and signing.
This form will also help the scheme to assess whether you qualify for advanced hepatitis C payments.
