Transfer a list of patients

When a dentist transfers their list of patients to another dentist, it’s called a bulk transfer. You can transfer a whole list of patients to another dentist or just part of it.
How to bulk transfer a list of patients
Request a GP209 form from your local health board and submit the completed form to our Customer Administration team (unless a local NHS board has requested it).
Email the completed GP209 form to nss.psd-customer-admin@nhs.scot
Transfer patients who aren’t part of your practice
If you’re requesting a transfer of patients who are not part of your practice, you’ll need to complete a GP208 form.
You should request this form from your local NHS board and return it to them.
