How to change patient details

Patient demographic details are used throughout the NHS in Scotland. These include:

  • name
  • address
  • date of birth
  • gender.

Patient details are kept up-to-date on the Community Health Index (CHI) using updates from GP practices and other sources.

Keep details current

Changes can be made to correct an error or to update a patient record with new information by following the steps below:

  1. If a patient informs you of a change, please record this in the practice clinical system and send it to us.
  2. Our team will update the CHI.
  3. Once the CHI is updated, this information is shared with other CHI users such as National Screening Systems, Scottish Care Information (SCI) Store and others.
