What to do if a patient wants their medical records

When a patient requests access to a medical record, it is subject to a clinical review.

To ensure this happens, the patient needs to make the request to the current or last GP practice where they were registered.

If your practice receives a request and you do not currently hold the record, it can be requested from us and we will retrieve and return it.

In rare cases, if your practice is unable to provide access, the request may be forwarded to the NHS board medical director for approval. This may happen where there is an ongoing legal case, for example.
