How to set up an eOphthalmic account

To access the eOphthalmic system, please complete the relevant application forms:

  • username and password set up
  • personal identification number (PIN)
  • acceptable user policy.

How to complete the username and password form

When you select the form it will open up as a PDF. You can complete the form on-screen and save a copy for your records.

The form contains two parts and you must complete all fields marked with an asterisk (*).

Contractor details – this part is for opticians or practice staff who can sign off claims for payment.

Practice staff – this part is for support staff who will only be entering the information on the webform and not actually submitting any claims.

Once you complete the appropriate part, print the form to sign and date it, as we can't process unsigned forms. Send the completed and signed form to us at:

You will receive your username and password in two separate emails.

You will then be ready to log‐on to the eOphthalmic portal and start completing the web forms.

Resetting the webform password

The following steps will help you reset your webform password:

  • Once the secure connection has been accessed (VPN or Tunnel), you will see the usual login screen.
  • The next screen is the homepage and the message in red will advise you to set five security questions. The answers to these questions will validate your identity when resetting your password and are case sensitive.
  • Click on the maintain security questions link. Once you set the five security questions, you can close this screen and process claims as normal.
  • The login screen now has a link named (Unable to login), so click that if you cannot remember your password.
  • You will now be asked for your username.
  • Fill in the answers to your security questions. Remember, the answers you give are case sensitive.
  • If you get a number of answers wrong your account will become locked and will need to be unlocked by the customer services team. They will advise what to do next.
  • You'll be asked to create a new password using the criteria detailed on this screen. Once the new password has been entered press the 'Set new password and login' button.
  • The logon screen will appear and you can logon as normal and start processing claims.

How to apply for a PIN

Download the PIN form. You can complete the form on the screen and save a copy for your own records.

The PIN form is used only when you need to sign off any ophthalmic claims. There are some restrictions around who can sign off what form.

There's a section in the form labelled 'Contractor Details'. This is for opticians or practice staff who can sign off claims for payment.

After you complete the form, you need to print, sign and date it. We won't process an unsigned form.

What happens after completing the form

Once we receive the form, a PIN will be generated in our system.

If you emailed the form to us then you'll receive your PIN by email, but if the form was sent by post then you'll receive a letter.

You are then ready to start using your PIN to sign off completed claims.

Signing off claims

You can sign off claims on the web form or from your practice management system (PMS).

Once a PIN has been authenticated, the claim will be sent through to us.

Sign the acceptable use policy (AUP)

All users must sign an acceptable use policy (AUP) to claim for payments online.
