GP Principals

Annual Certificate of Pensionable Profits Guidance Notes issued by Practitioner Services annually to provide guidance on the completion of the Annual Certificate of Pensionable Profits. This is subject to change each year and these guidance notes are aimed specifically at that year's certificate only

Annual Certificate of Pensionable Profits Form enables GPs to submit their Annual Certificate of Pensionable Profits annually.

Notification of Estimated Pensionable Profits Form should be completed and returned to Practitioner Services to notify an estimated value on which superannuation contributions are to be calculated.

Locum Superannuation Forms & Guidance

NHS Pension Scheme GP A Form GP Locums monthly certificate to advise Practitioner Services of NHS pensionable earnings.

NHS Pension Scheme GP Form A – Guidance for completion of the GP Form A for GP Locum NHS employment.

NHS Pension Scheme GP B Form GP Locums or out-of-hours (OOH) practitioner’s monthly record of NHS Locum or OOH earnings received.

NHS Pension Scheme GP B (Supplement) Form should be used to advise Practitioner Services of Money Purchase Additional Voluntary Contributions (AVCs)

NHS Pension Scheme GP Form B – Guidance for completion of the GP Form B used to record all earnings for NHS Locum and non-pensioned out-of-hours (OOH) work received in a month.

NHS Pension Scheme GP Solo Form should be completed by NHS General Practitioners in Scotland, for NHS work that does not fall within the GP’s normal GMS/Section 17c contract obligations.
