When to report an incident to us

You need to tell us if something goes wrong with the medical devices, social care equipment or estates and facilities you use. We call this an adverse incident.

An adverse incident is an event that causes (or could cause) harm to the health and safety of patients, users or others. This includes near-misses. By telling us about an incident, or a near miss, you can help us to avoid the same thing happening to anyone else.

Medical devices include:

  • syringes
  • surgical instruments
  • defibrillators
  • radiotherapy equipment

Social care equipment includes:

  • shower chairs
  • gripping aids
  • stair lifts

Estates and facilities equipment includes:

  • electrical wiring
  • water supplies
  • lighting and fire detectors
