Report an incident

When to report an incident to us

You need to tell us if something goes wrong with the medical devices, in-vitro diagnostic medical devices, estates, facilities, social care equipment and personal protective equipment (PPE) you use. We call this an adverse incident.

An adverse incident is an event that causes (or could cause) harm to patients, users or others. This includes near-misses. By telling us about an incident, or a near miss, you can help us to avoid the same thing happening to anyone else.

Medical devices include:

  • syringes
  • surgical instruments
  • defibrillators
  • software
  • X-ray machines

In-vitro diagnostic medical devices include:

  • test kits
  • reagents
  • blood analysers
  • software
  • calibrators

Social care equipment includes:

  • shower chairs
  • gripping aids
  • stair lifts

Estates equipment includes:

  • electrical wiring
  • water supplies
  • lighting and fire detectors

Personal Protective Equipment (PPE) includes:

  • FFP3 respirators
  • visors
  • gloves
